Gentlemen:

After 18 months we are closing further comment on this column. We encourage you to continue your conversations within the social network, where there is a specific discussion group on HIFU which we encourage you to use and which does not require me to approve your comments every time you post them.

We would also point out that your discussion on the social network will be far more visible to far more people than the discussion you are having here, which would therefore increase the visibility of your personal opinions on HIFU.

Ron, careful on the side effects. All treatments do experience a % of side effects including HIFU. I do think HIFU is the most least likely of serious side effects and definitely the least invasive of treatment choices. I have had my share of scaring problems with HIFU. Even with the problems I experienced I would do HIFU again. I hope the FDA gives final approval. The male population needs this choice of treatment in the good old USA.

Yes Scott, who can put a price on quality of life? Surgery, radiation or ultrasound ... what a question ...

SteveE, another undetectable PSA! Right on!

I understand that there are new machines. The Ablatherm has incorporated a manual override and the Sonablate has a computer that "tells" the doctor if all the tissue is gone; the two of you are proof that it works and works well.

I thank the doctors who are the pioneers and left the USA to continue progress with HIFU. Sitting around waiting for clinical trials would have just set the technology back. Oh yes, that's just what they wanted.

Side effects or scare tactics, I have several friends who had HIFU, they had/have no side effects.

Scott,

Your story is the best I've ever heard. HIFU just gets better by the day. I hope that you post this at other cancer forums. More of us need to know that HIFU is an option, and well worth the money.

Many congratulations!!

I had the HIFU procedure performed almost 1 year ago with Dr. Scionti in the Puerto Vallarta Hospital. The procedure was easy relatively painless, I had no issues with incontinence and was able to have erections 3 days later without the use of the prescribed Cialis. The technology is making huge strides and Dr. Scionti, being on the forefront, receives the latest software upgrades for the Sonoblate 500. The software has been able to accurately map out nerve bundles, bladder neck and deliver the prescribed joules of energy to the treatment zones for some time now; however, the latest upgrade provides the doctor with information on ablation at the cellular level with the addition of a Doppler feedback loop. Dr. Scionti that stated with the new software the patients have been consistently reporting undetectable PSA levels. My personal PSA has been undetectable from the 3- to the 12-month marks. My grading was T1c Gleason (3 + 3), with a 22 cm prostate. I had this procedure at 51 years old. As to whether Albatherm or Sonoblate machines are better, I believe the surgeon associated with this site, stated something like, choose your Tiger Woods and allow him to select his clubs. I believe this would hold true for radical prostatectomy of HIFU.

My PSA is now at 0.67. A little rise is normal. It's almost two years, and my PSA has been around 0.6 for quite sometime.

Someone mentioned being afraid of going offshore for treatment, but International HIFU only uses the most perfect hospitals, and they send their own nurses, anesthesiologist, and a certified mechanic along to monitor the machine. It's a top notch operation. Plus, you are guaranteed several American doctors in attendance learning the technique. The only foreign thing is the taxi to get you to the hospital, the nurse and a driver take you back to your hotel; they book The Atlantis in Nassau or the Westin in Puerto Vallarta.

Be great to hear updates from the rest of you.

Yes! John it is really great to hear from an old timer! Was your PSA 0.5 right afterwards, or did it wander around for a while?

Lucky guy Steve, that you didn't need the TURP. Hope you didn't wait too long to start on the Cialis, maybe increase your dosage if it isn't responding, eh? Get the blood to flow.

The FDA has a "team of advisors," not a "board of directors", but they are doctors, and they make the decisions.

Great to hear your news, sounds like you are quite lucky to be okay. Are you taking Cialis? From what I understand it's important to keep the blood flowing and the veins open, don't let them stay collapsed.

Did you have a TURP? I understand it's au-regular with the Ablatherm, did that give you any pain or trouble? It's given at the same time as the HIFU, right?

He "took" the right nerve bundle? Was it treated with HIFU, or surgery? I didn't know they could go outside the gland like that. Man, this HIFU can do anything.

Terrific PSA, I'll bet you feel like Shaun White feels today!

Just to let every one know my 3 month PSA came in at less than 0.1. The only change I made was I went with Dr. Orovan in Toronto, Canada. Very kind doctor and high skill level with the Albatherm. Now I just need to keep the PSA at this level. Did HIFU on October 22, 2009. I'm very pleased with the results; no incontinence; however, the right nerve bundle was taken for better coverage to increase the chances of removing any cancer that might have been near the nerves. This was discussed before the procedure. I am having a problem with ED, but I feel it will get better as each month passes.

HIFU works and works well. Dr. Suarez did the procedure on me almost 4 years ago. My PSA is 0.5 and I have a great sex life. It helps to watch your diet and take some immune boosting supplements for life after any prostate procedure. Sadly I got this disease in my late 40s. Hopefully the FDA will do the right thing and approve HIFU in the US and give guys another choice in the battle vs. cancer..
