Terminal prostate cancer, quality of death, and hospice care

The role of hospice care and its potential benefits — in terms of quality of life and quality of death – for men who are actually dying of prostate cancer is theoretically well understood but relatively little studied.

Is Trofex a possible “replacement” imaging agent for Combidex?

According to a media release issued on Thursday this week by Molecular Insight Pharmaceuticals, Inc. (MIP), an investigational imaging agent known as Trofex™ (a.k.a, 123I-MIP-1072) may offer a new opportunity to identify the presence of early signs of metastatic prostate cancer in the prostate bed, soft tissues, and bone within 4 hours after injection.

XMRV and prostate cancer — a brief update

We have previously reported on studies related to the possibility that the xenotropic murine leukemia virus–related (XMRV) virus is in some way involved with risk for prostate cancer in some patients.

ADT-associated weight change in the real world

Initiation of androgen deprivation therapy (ADT) has long been associated with the loss of lean muscle mass and a gain in mass of fatty tissue, leading toward an overall weight gain over time. However, data from the “real world” on the degree to which ADT actually affects body mass has been very limited.

Outcomes from a “watchful waiting” protocol for localized prostate cancer

A new study reports on retrospective analysis of data from a series of 218 patients diagnosed between 1991 and 2005 with localized prostate cancer and who were managed using a “watchful waiting” (as opposed to an active surveillance) strategy at the Massachusetts General Hospital (MGH).

Is MSMB the next great thing in prostate cancer testing?

If you just read the media reports (for example, from Reuters, from the BBC or from The Daily Telegraph) you might get the idea that a new urine-based test for a protein called MSMB is vastly better at detecting prostate cancer than the PSA test. Sorry, but that is not actually the case at all.

Satisfaction after surgery for localized prostate cancer

There really are remarkably few published data from well-constructed studies of patient satisfaction following treatment for prostate cancer presumed to be localized at the time of treatment.

Chemohormonal therapy + EBRT for treatment of locally advanced prostate cancer

Several years ago, Bolla et al. planned and carried out the major European trial showing that external beam radiation therapy with 3 years of androgen deprivation therapy (ADT, using goserelin acetate)  extended the prostate cancer-specific and the overall survival of patients with high-risk localized prostate cancer as compared to radiation alone.

Prolaris: prognostically accurate but of limited utility?

According to a media release from  Myriad Genetics, the company’s investigational genetic test (Prolaris™) can accurately predict prostate cancer-specific mortality at 10 years in men known to have clinically localized prostate cancer.

Clinical implications of an initial diagnosis of ASAP

Relatively few data have been published on the pathological outcomes of patients initially diagnosed with atypical small acinar proliferation (ASAP), i.e., atypical glands suspicious but not diagnostic for prostate cancer, who are diagnosed with prostate cancer on re-biopsy and then treated by radical prostatectomy.

“Compassionate use” trial of abiraterone acetate in mCRPC to start soon

As previously reported, a “compassionate use” study of abiraterone actetate is expected to start shortly (first in the USA and then in other countries).

PSA-based triggers for treatment in active surveillance protocols

A new report from Laurence Klotz’s group in Toronto basically tells us that no currently identified PSA-based “trigger” can be used alone to determine when a man on active surveillance should be switched over to active treatment.

Abiraterone acetate extends median survival by 3.9 months in mCRPC

The results of a randomized, double-blind, placebo-controlled, multi-center, Phase III clinical trial comparing abiraterone actetate + prednisone to a placebo + prednisone in men with metastatic castration-resistant prostate cancer (mCRPC) were presented today at the annual meeting of the European Society for Medical Oncology in Milan, Italy.

CEASAR study to compare effectiveness of first-line therapies for localized prostate cancer

According to a media release last Thursday and a report on Medical News Today, the Agency for Healthcare Research and Quality (AHRQ) has awarded a large grant to the Vandebilt-Ingram Cancer Center) for study of the comparative effectiveness of differing types of first-line treatment for localized prostate cancer.

ADT, bone architecture, and “virtual bone biopsies”

A recent study reported in the Journal of Clinical Endocrinology and Metabolism has been receiving a good deal of media attention, although its immediate clinical importance wouldn’t appear to be particularly high.
